The Cockcroft-Gault formula estimates creatinine clearance (CrCl) using serum creatinine, age, weight, and gender: CrCl (mL/min) = ((140 - age) * weight) / (72 * SCr) multiplied by 0.85 for females.

Loading dose is typically 25-30 mg/kg of actual body weight. For mild infections (target trough 10-15 mg/L), use 25 mg/kg. For serious infections (target 15-20 mg/L), use 30 mg/kg.

Dosing intervals depend on renal function: CrCl ≥60 mL/min: q12h; CrCl 30-59: q24h; CrCl 15-29: q24h; CrCl <15: q48h or per further adjustment.
